There are ten public road access points to the city of Middletown.  Because of a combination of dense traffic, multiple lanes, and stiff grades, SR-4 south through Amanda and SR-122 east at the I-75 interchange are not much used for cycling.  The remaining eight access points are all cyclable, though some are more bike-friendly than others.  The following table shows the approximate geographical relationship of these access points, the top of the table representing north.

Trenton-Franklin Road (southwest / northeast) is nearly flat, but has no appreciable shoulder, and experiences moderate traffic through Poasttown.  Thomas Road, about one mile to the north, offers a quieter but hillier alternate route around Poasttown traffic.  SR-4 continues north to Germantown and beyond; however, Middletown-Germantown Pike, which can be accessed north of the railroad bridge, is a more pleasant ride.

SR-73 continues through Franklin.  It connects with SR-123, which is reasonably bike-friendly heading northwest to Carlisle and Germantown, but serves heavy traffic (including trucks) southwest toward Red Lion.  Other bike-friendly routes into and out of Franklin include Dayton-Oxford Road (northeast / southwest), Lower Springboro Road (east), and Union and Shaker Roads (south).  Dixie Highway (north / south) is also ridable, but has somewhat heavier traffic, and no shoulders south of Franklin.

Manchester Road (2 lanes) continues east to Union, Shaker, and Robinson Vail Roads. Dixie Highway extends north to Franklin and south to Blue Ball, where it becomes Cincinnati - Dayton Road continuing southward to Monroe and beyond.

Between I-75 and Union Road, Manchester Road crosses a gully with steep grades on both sides.  When travelling in either direction, it is helpful to maximize speed on the downgrade in order take advantage of momentum on the climb.  The road surface is usually clear, but be alert for deer.

SR-122 continues west to Gratis and Eaton.  It is hilly and serves moderate to heavy traffic at speed.  Trenton-Franklin Road is nearly flat, extending northeast across SR-4 to Franklin, and south to Howe Road.  Howe Road is hilly; it leads west to Jacksonburg Road via Miltonville and (via Elk Creek Road) to Trenton.

Union Road extends north to Franklin, and south to Nickel Road east of Monroe.  Hendrickson-Bigler Road continues east to Shaker Road.

On Caprice there can be long waits at the Cincinnati-Dayton Road stop sign.  Also, a half-mile long, unshaded 10-percent (est.) grade makes Hendrickson-Bigler a challenging climb, especially in hot weather, and surface irregularities make descending at more than 30 miles per hour risky.  Eastward from Union, however, the road's profile becomes considerably tamer.

Though bicycle-legal, SR-122 at the I-75 interchange is made extremely hazardous by a combination of multiple lanes, ramps, grades, and severe traffic congestion.  It is therefore undesirable as a bicycle route.

Because of the hazards of SR-122 and the severe grade on Hendrickson-Bigler, most cyclists prefer to make connections east of Middletown using Manchester or Greentree Roads, via the Manchester and Turtle Creek Township access points, respectively.

Todhunter Road extends west to SR-4, east to the southern continuation of Yankee Road, and further east (over steep grades) to Cincinnati-Dayton Road.

Some riders may prefer to use the Middletown portion of Yankee Road only on weekends and holidays, when industrial traffic is minimal.
